1095-C forms have been processed for benefit eligible employees. If you have provided consent for electronic 1095-Cs, you can view/print your 1095-C for your records now. Simply log into Self Service  https://selfservice.evansville.edu and click on "Tax Information." You will see the option to receive the 1095-C form. 1095-Cs have been mailed for those employees who did not provide their electronic consent.
